Diabetes is a disease that affects over 16 million Americans. It is the fourth leading cause of death by disease and the leading cause of kidney failure, blindness and lower leg amputations. The incidence of heart attack and stroke in individuals with diabetes is two to four times that of the general population. The incidence of diabetes is steadily increasing and will affect nearly 20 million Americans by the year 2002.

Age is a significant contributing risk factor for the development of diabetes. As Americans live longer, the prevalence of diabetes is expected to increase. Other risk factors are family history, inactive lifestyle and obesity. Although we can do nothing to alter our genetic history or our age, the choices we make as it relates to our lifestyle can have a significant impact on whether we develop diabetes as well as how to best manage the disease. Three key factors in prevention and management of diabetes are eating a healthy diet, maintaining a desirable body weight, and staying active. In fact, this "lifestyle prescription" is the mainstay in successful management of diabetes.

Solutions For Better Aging's Diabetes Drug Review is a reference intended for the purpose of determining the appropriate use of diabetes medications in the elderly. It should be noted that AgeNet promotes the American Diabetes Association Standards of Care for the Management of Diabetes. In short, it is important to follow these standards of care since medications alone should not be relied upon. Over 95% of those diagnosed with diabetes have Type 2 diabetes, formerly known as Non-Insulin Dependent Diabetes Mellitus or NIDDM. This is the most common form of the disease in the elderly. Our discussion on the appropriate use of diabetes medications in the elderly will focus on Type 2 diabetes.

Drug selection in the elderly is important since some medications pose an increased risk of adverse effects or may not be as effective depending upon patient characteristics. One of the initial determining characteristics in choosing the most appropriate medication is whether the person is obese (>20% over ideal body weight). Typically, the root cause of high blood sugar in the obese patient is insulin resistance. There are newer medications on the market which are more effective in treating insulin resistance than medications previously available. Of the three available drugs (Avania, Glucophage and Rezulin) which affect insulin resistance secondary to obesity, AgeNet recommends Avandia as the drug of choice. Glucophage is relatively contraindicated in the elderly due to an increased risk of developing lactic acidosis and Rezulin (NOTE: Rezulin was voluntarily pulled from the market on March 21, 2000) has a higher risk of adverse effects on the liver. This is not to say that Glucophage and Rezulin are not appropriate. The selection of a diabetes medication is highly individualized and Glucophage and Rezulin do have their place.

Since there are a large number of non-obese and frail Type 2 elderly with diabetes, other options need to exist other than the above three (3) medications. This is due to the fact that insulin sensitizers are not as effective in non-obese people with diabetes. In these cases one of the safer oral sulfonylureas, or oral hypoglycemic agents, is preferred. Solutions for Better Aging lists Glucotrol (glipizide) as a preferred agent in the non-obese Type 2 person with diabetes. This agent increases the release of insulin from the pancreas. Other oral agents listed as non-preferred or excluded are listed as such primarily due to their longer duration of action in the elderly which increases the risk of serious or life threatening hypoglycemia, e.g. Diabenese (chlorpropamide). Cost versus benefit is also a factor in determining placement of an oral agent on the formulary.

When mono-therapy with one of the above agents is approaching the maximum daily dose and the therapeutic response is inadequate, combination therapy is an acceptable next step. Examples of combination therapy are:

1. Oral hypoglycemic agent (Glucotrol, Diabeta, Micronase, etc.) and Glucophage.

2. BIDS therapy, which stands for Bedtime Insulin and Daytime Sulfonylurea. Adding a small dose of an intermediate acting insulin at bedtime, e.g. NPH or Lente insulin, is effective at lowering early morning rises of blood glucose while daytime administration of the oral agent works on daytime glucose excursions.

3. Addition of Precose to an oral agent works (Glucotrol, Diabeta, Micronase, etc.) just prior to mealtime when after-meal or post-prandial high blood sugar occurs. This agent works by lowering the rise in after meal blood glucose excursions.

4. Adding Rezulin, and perhaps Avandia, to someone solely on insulin may reduce insulin requirements and, in a certain number of patients, eliminate the need for insulin altogether.

In all the above cases, combination therapy tends to have an additive effect at lowering blood glucose. Other regimens of combination therapy are less well studied yet are being used. Please note: When mono-therapy fails on initiation of treatment or over time, insulin may need to be started since glycemic control is paramount in reducing the likelihood of developing long term complications and in improving quality of life.
